Hazel House is a residential home in a small village just before Grantham centre. The home offers specialised and professional support for young adults aged from 17 years with learning disabilities, autism and other complex needs and diagnoses.
Hazel House is a bespoke and tailor made service made to fit each individual’s needs, treating everybody with dignity and respect, seeing each individual as the individual they are.

We are a diverse and professional team who pride ourselves on working as one team to empower each other, ensuring all the young people we care for feel valued and respected. Hazel House staff aims and goals are to support individuals transitioning from schools or other placements into adult services offering 24/7 care and support. Through ensuring that Hazel House provide clear structure and continuity in all aspects of care and life skills the staff teach, the home will be able to produce positive outcomes for the individuals.
Personal development is accomplished in many different ways and at individual rates. In order to ensure that each individual achieves their full potential and in the direction that the individual prefers, all care plans, programs and personal targets are devised. This is done by using person centred approaches, through regular person centred planning meetings held with the individuals, to which their own contribution to developing plans is paramount.
There is a wide range of leisure, recreational and social activities available for the individuals we support to pursue whilst at Hazel House, alongside building their life skills. The home is located close to Grantham centre, which provides a number of facilities for the individuals we support. Staff encourage the young people to participate in communal and group activities of their own choice.
These activities include, cinemas, bowling, swimming, horse riding, shopping, walks in national parks, cooking, arts and crafts and many more.
Care and Accommodation
Hazel House was purchased privately and was a large existing family home. The home has now been designed in many areas to meet the needs of the individuals, offering four bedrooms, which will be decorated to their personal tastes.
There is a large communal kitchen and breakfast area, with a separate dining area for those wishing to eat in a quieter environment. There is an arts and crafts area and a cosy corner sensory area with a large beanbag to relax.
Hazel House has a large communal lounge with lovely views of the garden, and a separate laundry/utility area so those who wish, can be supported with developing their daily living skills. There is also an enclosed garden with areas to relax with seating and BBQ area.
The home has its own minibus so the individuals can be supported on day trips, holidays etc.
